Please print out and fill out the Liability Waiver Form. Liability Waiver Forms can be e-mailed to ofa@onetimesports.com and/or can be handed in to the coach. All players participating in the tournament must fill out a Liability Waiver Form. ALL forms must be handed in at the entry gate before the start of the games.
